Student/Parent Tablet Agreement 2016/2017 for K-12 This Agreement is made between Guidance Charter School (GCS) and the (Student/Parent) designated below. RECITALS The purpose of this Agreement is to provide a technology package that includes the temporary use of a mobile tablet device, technical support, and the use of GCS licensed software. This Agreement is in addition to any other GCS/Computer / Internet usage policy. TERMS In consideration of the mutual covenants and conditions contained in this Agreement, GCS and the Student/Parent agree as follows: 1.0 Description of Equipment. GCS agrees to provide Student with a fully configured mobile tablet device; to include online version of students’ textbook materials. 2.0 Term As long as Student has possession of the mobile tablet device provided by GCS, they are bound by the terms of this agreement. 3.0 Consideration Student is enrolled in a tablet program during the 2016-17 academic year. There is no cost to the student or parent associated with the mobile tablet. 4.0 Use and Alterations Student agrees with the acknowledgement of parents to use the mobile tablet device in a careful and lawful manner and shall not make any alterations, additions, or improvements to the mobile tablet device. 4.1 GCS will maintain updates and providing technical assistance for any applications or hardware that is loaded for purposes of students’ educational needs and use of the tablet. 4.2 Student shall not remove or alter any GCS, and/or GCS’s Service Provider’s identification labels that are attached to or displayed on the mobile tablet device and associated peripherals Under NO Circumstance may students alter or upload personal data onto the tablet. Tablets will be scanned daily for any/all content. 5.0 Lost, Stolen and Damaged Equipment 5.1 Student shall take prudent care to maintain the mobile tablet device and assorted components is a safe and secure manner. The Student/Parents is responsible for the entire replacement cost of the mobile tablet device and/or components that are lost or damaged. 5.2 In the event that the mobile tablet device is stolen from the school by any student, the student/parent will be prosecuted and is required to pay full restitution cost. If the student owed money as a result of any damage charges; the school will not release any records, report cards or pertinent documents to the student/parent until the fine is cleared. 6.0 Assignment and Compliance with Applicable Law and Policy 20
